Hello Bradshaw Farm parents,

There was a new law created last year that affects our summer swim league.

It is the concussion/back to play law.

The law was created to add more consideration when bringing a child back to play after receiving a concussion.

The county is requiring two things from each of us.

#1 - INDIVIDUAL FORM YOU MUST SIGN - Parent/Athlete Concussion Info Sheet:

We have put on our team website homepage a form that you need to print out, sign, and give to us. We have to have this on file. It is also required for any swimmer who goes on to districts and state competition. If we do not provide a copy to the Cherokee Summer Swim Association (CSSA), your child will not be allowed to compete at districts/state.

Please do one form per child. If you have 3 children, please do 3 forms, etc.

#2 - MASTER SIGN OFF FORM YOU MUST SIGN - in person at our pool:

In addition, the CSSA is requiring a separate sign off form that we provide them as well.

This will be available for you to sign up at the pool.  It is a master list so to speak, so we can ensure everyone has read the document and provided us a copy. This master list will be given to the CSSA.
